Upload Proof of Vaccination

Frank Doran Shihan has mandated that everyone provide proof of vaccination before entering the dojo.

There are two ways to provide proof of vaccination:

1. You can securely submit an image using the web browser on your phone here:

2. You can email an image of your card to aikidowest@gmail.com

Please take care of this step in advance so that the beginning of class is not delayed.
